Having a cancer diagnosis is often a very complicated experience for any one individual to have to encounter. After the initial shock wears off, we often find ourselves facing a new set of challenges post diagnosis and suddenly find ourselves lost in a maze of health care professionals, institutions and family stresses.
Survivorship Care will ensure that each individual served and their loved ones, have the best advocate available to support them through the various challenges that may arise after a cancer diagnosis is confirmed along the continuum of care. We connect each of our patrons to the right people, places, opportunities, and things to help them accomplish their personal goals and related health care objectives before, during, and after a cancer diagnosis is confirmed.

What is a Care Manager?
A Care Manager (also referred to as a Care Coordinator, Patient Navigator, or Health Navigator) is a trained professional with working knowledge and experience, in the field of aging and health care. A dedicated Care Manager provides care coordination between the individual
(patient and family), their health care team and other community partners that are involved in the day-to-day during the cancer care journey. A Care Manager has experience in helping assist seniors, adults, disabled adults, children and their families with short and long term care arrangements, everyday practical concerns/issues, and advocacy as related to cancer care support and survivorship.
At Survivorship Care, your dedicated Care Manager is your personal cancer care concierge and will work diligently to provide each individual with a tailored plan that is meant to meet their own personal goals and support his/her ever changing daily needs in disease prevention, at diagnosis, during treatment, in survivorship planning, and through to end-of-life care.
What is the role of the Care Manager?
Care Managers build a supportive relationship of trust and respect for each individual and their families they serve. A Care Manager can help provide guidance and oversight in treatment planning, in care coordination, locating medical specialists, in arranging needed transportation, in connecting the individual and their family to the right community partners, in securing resources, and in coordinating care between various health care providers. Each Care Manager serves his/her patient from a place of compassion and encouragement, while staying actively involved during an often challenging and overwhelming time, when advocacy support is often needed most.
As a Care Manager and health care professional, our team can perform a home-based visit and complete a full assessment in order to identify any problems that may exist and recommend the possible solutions attainable.
Our Care Managers have referred their clients to supportive services such as:
Meals on wheels, home care, palliative care, home health, emergency safety solutions, mental health, eldercare attorneys, financial planners, registered dietician/nutritionist, medical equipment, obtaining wigs/prosthetics, real estate agents and other vital community support & service offerings.
As your dedicated partner in caring, we often act as a "connecting bridge" between the individual, their family members, friends, and the other various professional who may be involved in the day-to-day care of the person diagnosed with cancer. We ensure that our patrons have access to the best professionals in the health care industry and advocate for each and every person and their loved ones.
Last but not least, after our cancer care management and advocacy agency coordinates all the various support services needed, we keep in touch with verbal and written reports so the family member(s) are well informed about their loved ones status throughout the continuum of care.
